Are there any forms I need to fill out to go on a mission trip?
Please download (right click, Save As...) some of the forms we will need you to review before going out with FRM. If you intend on going on a missions trip, PLEASE BE SURE TO DOWNLOAD THE MISSION APPLICATION AND FILL IT OUT COMPLETELY AND CLEARLY.
